Popular local KC bakery is partnering with ModRN Health to provide concierge healthcare to their employees.

.Responsibilities:

• Conduct thorough holistic assessments of members virtually or at their place of employment in diverse locations.
• Develop collaborative and personalized care plans with members, emphasizing long-term relationships and understanding of personal health goals.
• Work closely with ModRN Health physicians to provide exceptional, evidenced based care for patients/members.
  
• Measure progress towards health outcomes and optimize members’ healthcare journey.
• Visit employee site, incorporating some travel.
• Serve as a role model in the nursing profession, maintaining the highest standards of professionalism, member courtesy, and confidentiality.
• Work collaboratively with the team to provide exceptional member service.
• Adhere to HIPAA guidelines and ModRN Health’s privacy policies.

Additional Responsibilities:

• Contribute to patient education initiatives, empowering members to make informed decisions about their healthcare.
• Advocate for members to ensure the right level of care is received at the right time for the best cost.

Essential Functions:

• Establish therapeutic relationships with assigned member population.
• Collaborate with members to prioritize and develop personalized, continuous care plans.
• Triage health issues, conduct routine care, health reviews, follow-ups, and consultations for various points of care.
• Provide pre/post appointment preparation and review, medication reconciliation, member education, and general triage.

Requirements:

• Excellent customer service, people skills and professionalism are a MUST.
• Active, unrestricted Compact Registered Nursing license in good standing (ADN accepted, BSN preferred).
• Preferred 7+ years recent Hospital/Clinical experience, especially in ED, Med-Surg, Cardiac, or Primary Care.
• Experience in care management, medical case management, or patient advocacy is a plus.
• Ability to speak Spanish (preferred, not required)
